WAIVER OF LIABILITY, RELEASE, AND INDEMNIFICATION AGREEMENT

IMPORTANT: READ CAREFULLY

I, the undersigned, being the parent or legal guardian of the minor child identified below (the “Participant”), acknowledge and agree as follows:

I understand and acknowledge that yoga and related movement activities, including but not limited to physical postures, breathing techniques, games, and mindfulness practices (the “Activities”), involve inherent risks, including but not limited to personal injury which may result from falls, physical exertion, contact with other participants, equipment use, environmental conditions of the premises, or negligence of instructors, other participants, or third parties.

By enrolling the Participant in any yoga classes, workshops, or activities offered by Sloga Yoga Solutions LLC (“Company”), I affirm that:

• The Participant is in good health and has no known physical or mental condition that would prevent safe participation.
• I assume full responsibility for any and all risks associated with the Participant’s involvement in these Activities.
• I agree to notify the Company in writing of any medical conditions, allergies, or special needs prior to the start of class.

Release and Waiver:

In consideration of the Participant being allowed to participate in the Activities, I hereby voluntarily release, waive, and discharge Sloga Yoga Solutions LLC, its instructors, agents, employees, contractors, affiliates, volunteers, landlords, and representatives (collectively “Releasees”) from any and all claims, demands, causes of action, losses, liabilities, damages, or expenses of any kind (including attorney fees) arising out of or relating to any injury, illness, or other harm to the Participant occurring on the premises or in connection with the Activities, whether caused by negligence or otherwise.

Indemnification:

I further agree to indemnify and hold harmless the Releasees from any and all claims, losses, or expenses, including reasonable attorney’s fees and court costs, arising out of or resulting from any claim related to the Participant’s participation, whether brought by the Participant, myself, or any third party.

Emergency Medical Treatment:

In the event of a medical emergency, I hereby authorize the Company and its representatives to obtain medical treatment for the Participant and understand that I am responsible for any associated costs.

Assumption of Risk:

I voluntarily assume all risks, known and unknown, associated with the Participant’s participation in the Activities, including but not limited to injuries or illness due to movement, environment, equipment failure, or negligence of any party.

PHOTO/VIDEO CONSENT AND RELEASE

I hereby grant Sloga Yoga Solutions LLC permission to take and use photographs and video recordings of the Participant for marketing, social media, website, advertising, or other lawful promotional purposes. These images may include the Participant’s likeness but not their full name unless separately and explicitly authorized.

I understand that I will not receive compensation for the use of the Participant’s image and waive any right to inspect or approve the final use. I release the Company and its representatives from any claims, damages, or liability arising out of or related to the use of these images.

TERMS & CONDITIONS

1. Drop-off/Pick-up Policy: Parent or guardian must check in the child before class and be prompt for pick-up. The Company is not responsible for supervision outside scheduled class times.
2. Class Environment: The Company reserves the right to remove a child from class if their behavior is unsafe to self or others. No refunds will be issued for removal due to behavioral issues.
3. Refunds & Cancellations: No refunds are given after the first class. Missed classes are not prorated or refunded but may be rescheduled at the Company’s discretion if space allows.
4. Weather & Emergency Closures: In the event of inclement weather or facility issues, class may be canceled or rescheduled. Notices will be communicated via email and/or text.
5. Medical Information: Parent/guardian is responsible for informing instructors of all allergies, medical conditions, or other needs that could affect participation.
